Certificate Ordering Instructions

Internet Orders:

note - I can only accept Credit Card orders via the internet

note - I do not sell blank Certificates

note - as a rule I do not require a copy of your DD-214 or DD215, however, if you are ordering a personal Award Certificate and I can not locate your name in the DOD Active Duty or Veterans database, I may not accept your order without a copy of your DD-214 or 215.

note - you do not need an existing Paypal account, nor do you need to set one up to purchase direct from the internet.

If you want to include additional information on your certificate, such as a brief description of the actual citation or additional unit info, you may do so using the 2 info boxes on the order form.

Each Box on the order form contains room for approximately 5 lines of text.

At minimum, you should include your Rank, Full Name, Award Date, Unit and Location at time of award.

Mail in Orders:

if you want to mail me an order that's OK, I accept Checks and Money Orders.

Process your order as though you were going to pay on-line, and once you've selected your certificates,

1.  click on the View Cart button

2.  print the View Cart Page

note - the View Cart page will have all the necessary information for me to process your order and will contain my mailing address and total cost including excluding shipping, I do not charge shipping for mail in orders because I do not have the processing fees that are related to credit card orders.

3.  Mail me your payment and the print-out
